How to Nominate an Alumnus/a
Online Nomination Form

About the Medal
The Medal, first awarded in 1933, recognizes alumni for distinguished service of 10 years or more to the University—including its schools, alumni associations, regional Columbia Clubs, and University-wide initiatives. Past medalists have inaugurated or improved alumni programs, provided visionary leadership of school-based and University-wide alumni associations, regional clubs and have organized important outreach initiatives that have fostered new and expanding relationships between alumni and the University, both here and abroad.

How to Nominate an Alumnus/a
Use the online nomination form to provide specific and detailed information about your nominee. We encourage all nominators to work with the deans, alumni directors, and CAA representatives from the School or Association of your nominee to compile complete information and seconding letters of support. Please note that the candidate is not to be informed of his or her nomination.The alumni medals will be awarded on commencement day.

To fill out a nomination form online, log in with your UNI and password and type your information online. You will be asked to provide information about the positions your candidate has held, when the positions were held, references, and a brief explanation of the nomination. Go to the form now!

If you are unable to fill out the form online, you may also download a PDF version of the nomination form and mail or fax it to the address on the form.
